WAIVERS AND ACKNOWLEDGEMENTS
By registering for this event, I/we acknowledge that I/we (or my Parents/Guardian if under the age of majority) have read, understood and agreed to all text and documents presented as part of the registration process and will execute any required documents prior to participating in this event.
In consideration of acceptance of this registration by Chinguacousy Curling Club ("the club"), I HEREBY RELEASE, WAIVE AND FOREVER DISCHARGE the club and each of its members, officers and employees FROM ANY AND ALL claims, demands, damages, costs, expenses, actions and cause of action, whether in law or equity in respect of death, injury, loss or damage to myself or property, arising or to arise by reason of my participation in the event, that has not been contributed to or occasioned by any negligent act, by omission or commission, of any of the aforesaid.
Helmets
Guardians are to supply a properly fitted and secured (as per the direction of the manufacturer) curling, skating, hockey, roller blade, skateboard or other CSA (Canadian Standards Association) approved helmet which is to be worn while in the curling rink.
Youth aged 11 years old and under are required to wear a helmet.
Youth aged 12 years old and older are not required to wear a helmet, however it is highly recommended.
Rowan’s Law
For participants under the age of 26 years old, I/we declare that I/we have read and understood a Concussion Awareness Resource for the appropriate age group (Age 10 & Under, Age 11-14, or Age 15 & Older) as required by Ontario Law for anyone under the age of 26 to be able to participate in organized sports.

Consent to Release Information, Privacy Policy and Anti-Spam Policy
I/we hereby consent to the use of the personal information provided to the Chinguacousy Curling Club administration. This information is intended for the sole use of Chinguacousy Curling Club and will not be shared outside the Club.
Media/Photography: Consent and Release form
The Chinguacousy Curling Club has permission to have photographs to be taken and to use the photograph publicly to promote the Chinguacousy Curling Club. I/we understand that the images may be used in print publications, online publications, presentations, websites, and social media.
